1. Submit our VAPA Online Membership Application (credit card payment required) - make sure to check the "Renewal" box on the application.

2. Download our Printable Online Application and mail to VAPA at:

For the printed version you will need the Acrobat reader installed on your computer to print the form. The reader is free and can be downloaded from the Adobe Web site.
